Just wondering if anyone has created or seen a plugin for a nice looking CSAT survey for web?

Your plugin was the first place I looked. I guess I was hoping someone had some pretty artwork like the sample.

Modifications can always be made. I’d have to look, but you may even be able to accomplish what you’re after by applying GraffitiStyles. If you’re interested, open a ticket and I’ll see what I can work up.
