I am about to submit my first iOS to the app store and am freaking out on providing the “App Video Preview and Screenshots”.
1st question: Is a video required?
2nd: What is the best way to create all the required screen shot sizes? (I do not have all those devices.)

No. It is better but not mandatory.
Do that with the simulator. Eventually, resize the pictures in a picture editor to fit the requirements.

The thing with the video is that you can make it later if you want. Pictures are more important. At any rate, there are so many apps in the iOS App Store that you need all the marketing you can get to have a chance to sell.
